We are pleased to announce updates to two of our New Business forms which include some minor modifications. Both forms are available for immediate use.
 
Face-to-Face Application for Life and Critical Illness Insurance Application Form (126E)
We have improved some of the sections in our Face-to-Face Life and Critical Illness Application Form by:

• Including a new section for multiple owners (4.5) and successor owners (4.6)
• Updating the regulatory sections (4.3 and 4.4) which now address all individually owned regulatory questions. This has resulted in an updated Verification of Identity Form (715E) – see below.
• Improving the usability of the Payments and Authorizations section (14)

The new paper version of this application (126E version 2019/03/01) can be ordered by contacting your MGA.

Verification of Identity Form (715E)
We have simplified some of the information required to complete the Verification of Identity Form (715E) and have also renamed it as the “Entity Verification Form” (715E version 2019/03/01). The updated form is available for immediate use and is also referenced in the Face-to-Face Life and Critical Illness Insurance Application. Copies will also be printed out with illustrations from The Wave (version 39.0), where appropriate.
 
Both forms are also available as fillable PDFs from our Advisor site under the Forms and Materials section.
 
SmartApp
Updates to SmartApp will be seamless and will not impact your in-flight cases.
 
Transition Rules
The updated Face-to-Face Life Insurance and Critical Illness Insurance Application Form 126E (2019/03/01) can be used immediately.  The current version of the form (126E version 2017/10/01) will continue to be accepted until further notice.
